Oceanside Pediatrics is here to help make this process as smooth as possible. Please complete & sign your part of the medication school form & either fax or drop it off at our office. It's important to ensure your child is up-to-date with their yearly physicals & asthma appointments as well.

That's why we offer Preparticipation Sports Forms to ensure they are up-to-date on their yearly physical and ready to play.
